JSC Forms and When to Use Them

The following forms are available in fill in PDF format. Enter your data into the forms then print them out. Mail the completed forms to the JSC Registrar, Catherine Snook, 3306 State Route 52, Stormville, NY 12582. Please keep copies for your records. The applicable fees may be paid by check (enclosed with the form) or securely online through PayPal. Any required photos may be mailed with the forms, or scanned and e-mailed to the Registrar (fernryan@aol.com). Once received, the photos will be forwarded to the certifiers for evaluation.

Certification Application NO FEE
Use this form when:
 • First submitting a sheep from an unregistered flock and/or unknown parentage
• When submitting lambs of sheep that are being progeny tested (all of the lambs from these breedings must be submitted and passed for the lamb to be used to register the parent)

Temporary Certificate fee $2.00 non transferable 

Registration Based on the Progeny Test Application FEE: $5.00
Use this form when:
• Requesting registration for adults that have completed progeny testing (You need all of the Certification numbers of the parents and/or the offspring to complete this application.)
• Requesting registration of the lambs of the adults who have completed the progeny test. (You need the Registration number of the parent to register the lamb)

Certification and Registration Application FEE: $5.00 
Use this form when requesting Registration for lambs that are offspring of JSC Registered sheep.

Breeding Lease Statement NO FEE
Use this form when you are breeding any sheep that is not registered in your name. This includes sheep in the same flock with different owners. Attach this form when submitting applications for registration of the lambs from this breeding.

Transfer of Ownership FEE: $2.00
Use this form when transferring ownership of an animal that is already registered. Return the original registration certificate with this form. Lambs sold before registration may be issued in the new owner's name, avoiding a transfer fee. This should be done by the seller of the sheep. 

 

Do not write on your Registration Certificates. All transfers and corrections are made by the Registrar. Please copy all forms for your own use.
